Note:  You can upload the items below into the application and/or bring them to the school if you are unable to upload the documents.

• Child's Social Security Number
• Child's Birth Certificate
• Child's Proof of Residence (Utility Bills)
• Child's Immunization Records (GA Form 3231)
• Child's Eye, Ear and Detail Certificate (GA Form 3300) 
• Parent/Guardian Driver's Licenses or Picture ID
• Copy of Custody Papers (If Applicable)
